The Greater Cincinnati Beacon Collaboration - Fostering better care, better outcomes and lower costs

Greater Cincinnati is setting the standard for health care in America through the Greater Cincinnati Beacon Collaboration. In September 2010, Greater Cincinnati was selected as one of 17 ONC-funded Beacon Communities that are building and strengthening local health IT infrastructure and testing innovative approaches to make measurable improvements in quality, cost and population health. Greater Cincinnati Beacon Collaboration partners supporting this program include HealthBridge, Cincinnati Children's Hospital Medical Center, the Greater Cincinnati Health Council, the Health Collaborative, the University of Cincinnati, Hamilton County Public Health, GE and dozens of area hospitals and physician practices.

The Beacon Community Program is funded by the Office of the National Coordinator (ONC) for Health Information Technology, part of the U.S. Department of Health and Human Services.

Txt4Health Mobile Initiative

  • ED Admission Alerts – Provide alerts to ambulatory providers when patients use emergency services or are admitted to a hospital

  • Direct Secure Email – Support improved transitions in care and alerting
  • Consumer engagement technologies – Txt4Health initiative to engage patients with pre-diabetes and diabetes

  • Adult Diabetes Demonstration, Lead Organization: The Health Collaborative -  Practices participate in a Patient Centered Medical Home collaborative and a Diabetes Improvement Collaborative
  • Pediatric Asthma, Lead Organization: Cincinnati Children’s Hospital Medical Center - Two sub groups: 39 practices in PHO group, three  primary care clinics serving Medicaid patients
